It occured to me to also share a Pastoral Care Workshop that I designed for the Pastoral Care Team with which I walk (I have also Pay It Forward resource). I hope that this may prove of use. Feel free to make it your own!

Couple of things
It is sufficiently documented and complete with the links / pdf's, that I feel we could clone it at our congregation.
It is an area we feel a need as training in pastoral care, specifically in those areas has been overlooked.
It is a way to reach out to the community.
