Profile:
The Parish Nursing program in the South Sherman Neighborhood will provide Health Education, Advocacy, Counseling, and Health Resource information to the members of this community. 

The success of this program will have significant benefits to the recipients of the health services in our neighborhood and will also provide a template to address specialized health education and service needs throughout Hamilton.

A key component in Poverty Reduction is in helping people to live healthier lives, to have access to all of the health resources in the community, and to build self-esteem through the setting and achieving of personal goals.

The successful achievement of personal health improvement is a major source of improved self-esteem.

Position Summary:

  • An experienced Registered Nurse will spend 24-30 hours per week serving the community based out of an office at St. Giles United Church on Holton Ave. at Main St. E.  The RN will provide health teaching, health promotion, advocacy and counseling to the citizens of the South Sherman Community and be available to address the specific needs of specialized populations such as:  residents of Residential Care Facilities, participants in the STARS program operated at St. Giles by the Elizabeth Fry Society, women at risk of homelessness and in crisis at four of the women’s shelters in Hamilton, and to the elderly seniors in this community to assist with their ability to live safely at home and remain socialized in their community.

The RN will also provide leadership and planning of educational events such as health fairs at residential care facilities, schools, and long term care homes. 

 Scheduled Working Hours:
days, some evenings, some weekends.

   

Qualifications:
1. Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse

2.  Member of RNAO, preferred

3. Diploma or Baccalaureate degree in Nursing 
4. Current BCLS certification
5. Successful completion of a recognized Parish Nursing Certificate Program (within the past 5 years), or a plan for successful completion of the same

6.  Current knowledge of the health, psychosocial and emotional needs of the South Sherman community population

7. Recent experience and or knowledge of the care and support of women in crisis

8. An overall understanding of the social service structure in Hamilton Wentworth an asset.
9. Demonstrate knowledge, skill and ability in health teaching and speaking to groups

10. Demonstrated ability to document according to the CON standards and keep accurate records and statistics

11.  Must have own transportation.

12. Proficiency in both Official Languages would be an asset.
